Specific Award Guidelines and Selection Criteria

2005 HTN Award Nomination Form

Outstanding Intervention Program Award
Presented to a program in existence for at least 3 years which provides direct service youth in the area of services to pregnant and parenting adolescents. The award winner will be that program which provides extraordinary services that are: comprehensive in scope, innovative, linked with other appropriate public or private agencies, and possibly replicable in other agencies.
 
Outstanding Prevention Program Award
Presented to a program in existence for at least three years which provides direct service to youth in the area of pregnancy prevention. The award winner will be that program which provides extraordinary services that are: comprehensive in scope, innovative, linked with other appropriate public or private agencies, and possibly replicable in other areas.
 
State Coalition Award
Presented to a state coalition whose:
*   goals, objectives and action plans are based upon well defined problems;
*   political impact has been effectively demonstrated;
*   efforts of support and advocacy to pregnancy, parenting, prevention and/or care services are well
    documented as extraordinary; and whose
*   commitment demonstrates evidence of broad based involvement and increased awareness to adolescent
     pregnancy and parenting.
 
Local Coalition Award
(Same requirements as State Coalition Award above)
 
Outstanding Teen Parent
This award is presented to a former teen parent who has overcome the obstacles of very early parenting to achieve extraordinary personal and/or professional success. Typically, this person is at least 30 years old, self-sufficient, and demonstrates outstanding leadership in achieving community or career goals. This person may or may not be a HTN member.
 
Outstanding Individual Professional
Presented to an individual professional who has:
*     performed extraordinary work delivered with energy, ingenuity and compassion - an expression of
       commitment beyond the call of duty;
*     exhibited willingness to put reputation on the line, if necessary, to improve services available to
       adolescents;
*     been responsive to the needs of adolescents by cutting through "red tape" and developing more effective
       methods of service and delivery; and
*     demonstrated outstanding and reliable performance, both in crisis and under the formidable pressures
       of daily routine.
